PHQ-4 anxiety and depression check (PHQ-4)
Four questions combining the two core anxiety items (GAD-2) and the two core depression items (PHQ-2). A very short first check that points to the fuller GAD-7 or PHQ-9 when either part is raised.
Developed by Drs. Robert L. Spitzer, Janet B.W. Williams, Kurt Kroenke and colleagues, with an educational grant from Pfizer Inc. No permission required to reproduce, translate, display or distribute.

You can take it privately on PsychologyOrbit: PHQ-4 self-check. Answers are scored once and never stored. A screening result is not a diagnosis.
