PHQ-4 anxiety and depression check

Four questions combining the two core anxiety items (GAD-2) and the two core depression items (PHQ-2). A very short first check that points to the fuller GAD-7 or PHQ-9 when either part is raised.

This is a screening questionnaire, not a diagnosis. If you are in danger right now, contact your local emergency number or see the crisis page.

Over the last 2 weeks, how often have you been bothered by the following problems?

1. Feeling nervous, anxious or on edge
2. Not being able to stop or control worrying
3. Little interest or pleasure in doing things
4. Feeling down, depressed, or hopeless

Your answers are scored for this one page and are not saved anywhere.
